The entire spout should spin off (the chrome piece). Then soak it in CLR (see below)
If it won't come off get some CLR (can get it most anywhere) or if you have it more handy white vinegar may work), a plastic lunch bag and a rubber band. Put the CLR/Vinegar in the bag and rubber band it so the spout is fully in the liquid and let it sit. The calcium buildup (assuming it's calcium causing your issues) will disolve in either of them (CLR is just faster in my opinion). I normally will let it sit for a little while, then take the rubber band and bag off (carefully), run the water to rinse and repeat as necessary.
